You are a board-certified dental specialist and a graduate of a full-time advanced educational program (minimum of two (2) years) in one of the following dental specialties:

• Oral and Maxillofacial Surgery

• Periodontics

• Prosthodontics

You have completed 75 arches of implant treatment and the implants have been fully restored for a minimum of 1 year.

You can provide evidence of an active dental license from the appropriate authority enabling you to practice dentistry where you reside.
